answers
1) yes, but it is becoming less of an issue.

2) yes, just as long as the track title+album name+artist are unique before you rip. Best to get them as correct as you can.
3) OggVorbis/Best if you like to stay away from MP3, else use MP3/Best

hmmm
1) do those non-working discs play in regular CD players?
2) do they play in commercial karaoke hardware?
3) do they play in other computers? (do you have access to a different drive? maybe at the store?)
it could possibly be the drive? but otherwise i've never seen this.

different drives
different drives have different properties. there are some Sony DVD±RW drives that DO work, and some that work only partially (no error correction) and some that don't work at all. So you have to just try and see. Don't give up, you'll find a drive that works!

I'm so glad to hear it's working. Yes i've had troubles from every corner using USB devices, (reported from many users) but never had trouble using FireWire devices.
Thanks for letting us know!

me too
this is not really a problem with kJams, but apparently the OS. I had stuck a CD into iTunes (which, unbeknownst to me, had some goop on it) and started ripping it. When it got to the track covered in goop, iTunes just hung my machine. Forever. Forcing a hard reboot. got the spinny beach ball, no ot...

Re: nope
actually a good number of them (depending on the build run) have "HL-DT-ST DVDRW GWA4080M" drives, which can certainly read. I'd like to confirm that they can also write, tho I suspect they can. They all seem to have the MATSHITA drives

no, just switch back into kJams, and the list is updated. Make sure the last template has a carriage return after it!
